Sequoia Therapeutic Recreation is seeking a dedicated and compassionate individual to fill the role of Community Inclusion Specialist. In this position, you will be responsible for providing individualized services for adults with intellectual and developmental disabilities. Individual Services:- Developing, planning, and facilitating inclusive activities in the community for folks with intellectual and developmental disabilities- Reviewing each client's person-centered services plan and finding meaningful connections for them- Collecting data, creating SMART goals, writing progress notes and applicable summary reports on client outcomes following each specific contract template. - Maintaining regular contact with family/staff and case resource managers who support each client- Providing Life Skills* services that support people with I/DD to learn or maintain a range of life skills in one or more of the following 5 categories: Self-empowerment, Safety awareness & Self Advocacy, Interpersonal Effectiveness & Effective Social Communication, Coping Strategies for everyday life challenges, Managing Daily Tasks and Acquiring New Skills.- Providing Community Inclusion support that promotes individualized skill development, independent living and community integration so individuals can learn how to actively and independently engage in their local community. Community Inclusion activities should be client driven, in their local community, integrated, and be a place they can develop relationships and make a contribution. - Providing Community Engagement by helping individuals find community groups and activities that align with their interests. Assist clients through connection process including helping them to navigate and access transportation independently.CompetenciesIn addition to the specific responsibilities listed above, you will also be expected to collaborate and cooperate with team members to meet the needs of clients and the community. This may include performing other duties as assigned, including substituting for coworkers in group activities.Candidates are required to obtain the following necessary certifications independently before engaging directly with clients.First Aid/CPR and Blood Borne Pathogens certificationTo be successful in this role, you should possess the following qualifications:A demonstrated commitment to the mission, vision, and values of Sequoia Therapeutic RecreationStrong communication skills, both written and verbalExcellent organizational skills and attention to detailAbility to work well independently with minimal supervision willing to ask for assistanceAbility to work well on a team and take on a variety of tasksProficiency in Google Workspace and ability to learn software systems necessary for job dutiesAbility to maintain excellent data records for registration, attendance, and client service hoursWork environmentWeekly schedule is generally Monday through Friday 9am - 6pm. You will be setting the schedule based on your specific clients' availability and needs. Some weekend, and evenings may be required based on quarterly activities planned and unique community opportunities for your assigned clients.Due to the direct service nature of our work, most work (30-32hrs) is expected to be completed on-site directly with clients and about 5-10hrs a week will be completed remotely. The remote work will include research, calls, planning, writing progress notes, and preparing summary reports.Off site tasks, creating relationships and opportunities with community members and community organizations is required. Use of personal vehicles is eligible for reimbursement in accordance with state and federal laws and Sequoia Therapeutic Recreation policy.Minimum Education and experience2-4 years of experience working with individuals with intellectual and developmental disabilitiesAt least 18 years of agePreferred candidate will have a Bachelor's Degree in Psychology, Sociology, Social Work, Education, Child Development, Gerontology, Recreational Therapy, Nursing or a related field or a valid Life Skills Coach certification. The State will require proof of education to be an approved Life Skills* provider.Additional eligibility requirements- Willingness to flex schedule to fulfill organizational needs (occasional weekends, and evenings)Physical Demands (reasonable accommodations may be made to enable individuals with disabilities to perform these physical demands)Ability to lift up to 50 lbs Ability to stand for up to 8hrsAbility to sit for up to 8 hrsTravel requiredTravel will be required to meet clients in various community locations.
