Job Description

We are searching for a dedicated MTM (Medication Therapy Management) Pharmacist to join our dynamic healthcare team. In this role, you will play a crucial part in optimizing medication therapy for patients, conducting comprehensive medication reviews, and collaborating with healthcare providers to ensure the highest quality of patient care. The ideal candidate will possess a background in medication therapy management or a willingness to learn. Need to have a commitment to patient well-being, and excellent communication skills.

Preferred Qualifications:

PharmD degree from an accredited pharmacy program
Experience in Medication Therapy Management (MTM)
State pharmacy license in good standing
Proficiency in utilizing MTM platforms and tools
Strong analytical and problem-solving skills
Excellent communication and collaboration abilities
Certification in MTM services
Familiarity with electronic health record (EHR) systems
Dedication to ongoing professional development and continuous improvement
