The Registered Dietitian (RD) is a member of the Interdisciplinary Team (IDT) with both clinical nutrition and food service management roles. The RD will conduct nutrition assessments and follow-ups as needed to assess participants’ nutritional status and provide appropriate medical nutrition therapy (MNT), such as therapeutic and mechanically altered diets, oral nutritional supplements or enteral nutrition recommendations, dietary counseling, nutrition education, food security resources, and coordination of care with other IDT disciplines. The RD will support Child and Adult Care Food Program (CACFP) compliance by planning menus to meet requirements, conducting quarterly CACFP food service audits, and maintaining required CACFP documentation. The RD will support meal service by planning therapeutic menus, maintaining diet records, coordinating diet changes with the Food Preparation Coordinator, and conducting regular kitchen audits. The RD will conduct in-services as needed for the kitchen staff and/or IDT members to ensure accurate and evidence-based nutrition care practices. Additionally, the RD in the corporate role supports the Nutrition & Dietetics Manager over staff coverage, training, administration, project coordination, and regulatory compliance. The RD covers various PACE Registered Dietitian duties remotely or in-person for any of the AltaMed PACE centers, supports training, assists with Nutrition & Dietetics operations including data collection and analysis, report building, presentations, and compliance recordkeeping. The RD also supports program planning, CQI initiatives, and interdisciplinary committees to improve participant health outcomes and nutrition care services. Collaboration with discipline managers, QI, Nutrition & Dietetics department, and other PACE leaders is essential to address organizational goals and optimize care delivery.
