Now Hiring: RN Case Manager – Join a Leading Healthcare Team!

Location: Reading, PA 19601

Full-Time | Competitive Pay |26 Weeks Contract with Potential to Extend

Important: Case managers will cover 2 Skilled Nursing Facilities within 20-25 miles

• Shift: 8 hrs. days Shift, 8a to 5p, (Mon to Fri)
• Scheduled Hours: 40 Hours a Week .

Hourly Compensation: $50.00 - $55.00 Per Hour.

Job Duties:

• Ensure accurate and complete documentation and collaborate with the interdisciplinary care team.
• Assist the provider/team with various care coordination activities.
• Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits.
• Responsible for collaborating with the primary advanced practice clinician, (APC) and facility to identify and respond to patient change in condition.
• Schedule and participate in family conferences, team meetings, and team case presentations.
• Coordination of training, tracking, and compliance with quality measures.
• Assist in creating and conducting services for selected audiences.
• Assist Providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation.
• Assist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions.
• Assist in managing members during a short-term sub-acute rehabilitation episode.

What We’re Looking For:

• Active and unrestricted RN license for PA State or Multistate.
• Must have a case manager or care coordination experience.
• Experience in LTC or SNF is highly preferred
